WHAT TO BRING - Please complete the On-Line Forms prior to your consultation.
Clients should bring with them to initial consultation the following items:
1. Tax returns for the last two years. (Self-employed individuals should bring personal and business for these years).
2. County tax notice for all real estate owned by you;
3. All Paycheck stubs for last two months;
4. Proof of any Charitable Contributions;
5. Proof of date of receipt of tax refund;
6. Itemization of how tax refund was spent (if spent recently)
7. Chapter 13 Only - Business Questionnaire if self-employed;
8. Chapter 13 Only - P&L Statements if self-employed.
Advance completion of the forms is extremely useful to us and enables us to fully evaluate your financial situation and explain all bankruptcy and non-bankruptcy options available to you for your particular circumstances. We can then assist you in making a very educated decision regarding which course of action to take
